Thanks, this sounds not unlikely. In the stack dump of the crashes, most of the time, the saved pc value also seems to be located in the startup code responsible for switching from 48 to 125 MHz. I'll run some tests tomorrow.I've seen such things when overclocking the flash too much, or improperly put the scope probe on it.
It can be QSPI signal integrity,, sometimes it helps to clean the board if got dirty.
Eventually, try to reduce the Flash clock and see any difference.
